Most people know cholesterol. Very few people know homocysteine. And yet elevated homocysteine is one of the most consistently documented independent risk factors for cardiovascular events — and one of the most addressable through targeted nutrition.
What Is Homocysteine?
Homocysteine is an amino acid produced during the metabolism of methionine — an amino acid found in meat, eggs, and dairy. Under normal circumstances, homocysteine is rapidly converted into beneficial compounds through two pathways, both of which require specific B vitamins as cofactors.
When those cofactors are insufficient — specifically Vitamin B12, Folate (as 5-MTHF), and Vitamin B6 — homocysteine accumulates in the blood. Elevated plasma homocysteine damages the endothelial lining of blood vessels, promotes platelet aggregation, and accelerates arterial stiffness.
Why Indians Are Particularly at Risk
India has among the highest rates of Vitamin B12 deficiency in the world, driven primarily by the prevalence of vegetarian and vegan diets. B12 is found almost exclusively in animal products. A National Family Health Survey estimated that over 47% of Indian adults may have suboptimal B12 levels.
Folate (Vitamin B9) is the second critical cofactor. The active form — 5-methyltetrahydrofolate (5-MTHF) — is the form the body can directly use in the homocysteine methylation pathway. Many supplements use folic acid, which must first be converted to 5-MTHF — a conversion that is impaired in a significant portion of people with the MTHFR gene variant (estimated at 30–40% of the global population).
